Graham Healthcare Group is seeking a Registered Nurse (RN) Hospice for a nursing job in West Mifflin, Pennsylvania.

SUMMARY
Provide skilled nursing assessment, planning, and coordination of services in order to maximize the comfort and health of patients and families, under the general direction of the Medical Director and/or the attending physician. Educate contracting facilities and community members and meet with multidisciplinary team and groups to market hospice services.
ESSENTIAL DUTIES AND RESPONSIBILITIES
include the following. Other duties may be assigned as necessary.
• Assume responsibility for a patient/family caseload that includes the assessing, planning and implementation, and evaluation phases of the nursing process
• Initiate communication with attending physicians, multidisciplinary hospice team members, and agencies as needed to coordinate optimal care and use of resources for the patient/family
• Obtain data on physical, psychological, social, and spiritual factors that may influence patient/family health status and communicate that data in team meetings to formulate an Interdisciplinary Plan of Care
• Maintain up-to-date patient records so that problems, plans, actions, and goals are accurately and clearly stated and changes are reflected as they occur
• Coordinate physical care of the patient and instruct primary caregivers, volunteers, and Home Health Aides in providing direct care as appropriate
• Follow up as needed and appropriately with doctor’s office and clinical and management staff pertaining to any concerns regarding patient care
• Supervise and document Home Health Aide plan of care every two weeks
• Attend interdisciplinary team meetings to facilitate discussion of patient/family status in order to evaluate and revise the interdisciplinary team plan of care
• Share in providing 24 hour on-call support 7 days a week, as needed
• Provide appropriate support at time of death and period of bereavement as needed
• Participate in orientation and in-service training programs for hospice staff
• Represent the company in professional and community activities as requested
• Participate in CHPC Continuous Organizational Improvement Program activities
• Maintain a high level of competency, growth and development, including ensuring license and CPR are current and in good standing at all times
• Follow guidelines as indicated in employee handbook.
• Attend nursing meetings and quarterly educational in-services and complete yearly competencies
• Perform other related functions as requested
• Document accurately and appropriately on the laptop, uploading by midnight
• When appropriate, enter insurance authorization requests within 24 hrs. of initial evaluation and continue to enter additional requests at the end of each authorization period
• Ensure clients have prescribed medications and/or treatment supplies, updating file as changes occur
• Adhere to/assist with company outcome goals as indicated by QI/homecare compare tracking
• Participate in QA/OBQI initiatives with problem-solving via action items

• Perform other duties as assigned by supervisor
QUALIFICATION REQUIREMENTS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. • Associate degree from an accredited School of Nursing
• Experience with terminally ill patients is preferable
• Able to meet the physical demands of providing nursing care, ambulating patients, transferring patients, assisting with exercises, etc.
• Able to transfer patients as needed according to proper techniques of body mechanics to promote patient and staff safety
• Understand the Hospice philosophy
• Able to use good and sound clinical judgment
• Able to document accurately and appropriately
• Must hold, in good standing, a license as a Registered Nurse, issued by the state in which you work
• CPR certified
• Empathetic, non-judgmental, tactful, responsible, and organized
• Understand the team approach and be a supportive team member
CERTIFICATES, LICENSES, REGISTRATIONS
Must have and maintain in good standing professional license, certificate, or registration, as applicable.

Employee will be required to drive daily for meetings and/or visits to residential homes. Employee will work in varied residential environments based on assigned case load. While performing the duties of this position, the employee may travel by automobile and be exposed to changing weather conditions.

The low end of the range includes compensation related to salary and first year success bonus. The high end of the range is inclusive of productivity bonuses and additional shift work that is paid at a premium (i.e., after hours, weekends, on-call). *Compensation potential varies by market.
